Dear Parents and Caregivers of Kindy students,
You should by now have received a parent/guardian consent form for the Audiology Screening appointments for next week.
If you have not yet returned your form, and you would like your child to have a free audiology screening, please return it to your child’s teacher as soon as possible, so we can lock in a schedule for the UWA Team.
If you have misplaced your copy, I have attached one to this notice.
